Cash payment at time of service reduces that rate that is charged for your sessions.
$165 Initial Appointment
$145 Follow-up Appointment (each)
$550 4 session package (save $50)
$820 6 session package (save $70)
We bill your insurance directly and you will be invoiced for any remainder due which is your responsibility (such as copay, coinsurance, deductible). It is important to note that insurance does not cover missed appointments or late cancelations and that will be your responsibility.

We are preferred, in-network providers with the following insurance companies:

Medicare/Medicaid: We are not contracted with Medicare or Medicaid. However, I can provide you with a form to submit to attempt reimbursement for cash sessions.
Insurance Authorization: Because every insurance plan is different, we can’t be certain your plan will cover your specific nutrition counseling needs. To find out, please call your insurance carrier. This form will guide you in asking the right questions.
If your insurance/health plan requires prior authorization for nutrition counseling, please ensure you have this before setting up your appointment or you will be responsible for the balance owing.
